Protein is an essential macronutrient and a routine part of virtually every diet. For most people with migraine, most protein sources present no particular concern. But a specific subset of protein foods, primarily those that have been processed, preserved, or prepared in ways that alter their chemical composition, are among the most consistently reported dietary migraine triggers. Understanding which protein sources are implicated and why provides a basis for targeted dietary modification rather than broad restriction.
Processed meats are the most consistently implicated protein-based migraine triggers in both clinical surveys and the migraine dietary literature. Hot dogs, bacon, ham, salami, pepperoni, bologna, sausages, and other cured or preserved meats contain sodium nitrate and sodium nitrite, preservatives added to prevent bacterial growth and maintain the pink color of cured meat.
In the body, nitrates and nitrites are converted to nitric oxide, a potent vasodilator. Nitric oxide causes blood vessels to dilate, including the meningeal blood vessels whose dilation is associated with migraine pain. Research has shown that nitric oxide is directly involved in the initiation of migraine attacks, and clinical trials using nitric oxide donors have reliably triggered migraine attacks in susceptible individuals. This places nitrate-containing processed meats among the most biologically plausible dietary migraine triggers.
The clinical experience is consistent with this mechanism. Surveys of people with migraine find processed meats among the most commonly reported dietary triggers, and the onset of attacks following processed meat consumption often occurs within hours, consistent with the time course of nitric oxide-mediated vasodilation.
Processed meats also accumulate tyramine as proteins break down during curing, smoking, and storage. The combination of nitrate content and tyramine accumulation makes preserved meats doubly relevant for people with migraine sensitivity to either compound.
Chicken liver, beef liver, and other organ meats contain high concentrations of tyramine. Organ meats are not typically consumed in large quantities in most modern diets, but when they are consumed, their tyramine content is high enough to be relevant for tyramine-sensitive individuals.
Smoked fish, including smoked salmon, smoked mackerel, and smoked herring, similarly accumulates tyramine during the smoking process in addition to containing nitrates from curing. Pickled herring and other preserved fish preparations appear in migraine trigger literature for similar reasons.
Sardines and anchovies are frequently mentioned in migraine dietary guidance. Both are oily fish with high histamine content, particularly when canned or preserved. Histamine accumulates in fish when bacteria break down the amino acid histidine after the fish is caught and before it is processed. Poorly refrigerated or improperly stored fish can accumulate histamine to levels that are clinically significant.
For people with reduced diamine oxidase activity, sardines, anchovies, and other histamine-rich preserved fish can lower the migraine threshold through histamine-mediated pathways distinct from those activated by tyramine or nitrates.
The key distinction across all protein categories is the degree of processing and preservation. Fresh, unprocessed protein sources including fresh chicken, fresh beef, fresh pork, fresh fish, and eggs are not commonly implicated in migraine and can be consumed without concern by most people with the condition.
The problem is specific to meats and fish that have been cured, smoked, dried, pickled, or preserved with nitrates. The chemical changes these processes introduce are the relevant variable, not the protein category itself.
The practical implication is straightforward. Fresh, unprocessed protein is not implicated and should not be restricted. Processed and preserved meats, including those found on pizza, in sandwiches, at breakfast, and in restaurant dishes, are worth evaluating as potential contributors in people whose attack pattern has not been fully explained by other triggers.
Systematic tracking is the appropriate method. Logging processed meat consumption alongside attack data over several weeks reveals whether a consistent temporal relationship exists. If it does, reducing or eliminating processed meats and replacing them with fresh protein sources is a targeted dietary modification with no nutritional cost and the potential for meaningful reduction in attack frequency.
