Water is not a migraine medication. Stating that plainly is important because hydration advice can veer quickly into the territory of oversimplified wellness claims that do not accurately represent what the evidence shows. Drinking more water will not prevent every migraine attack, and dehydration is not the cause of migraine in the way that a pathogen causes an infection.
What the research does suggest is more nuanced and more useful than either extreme. Adequate hydration appears to meaningfully reduce attack frequency in people who are habitually underhydrated. Mild dehydration lowers the neurological threshold for migraine in ways that are biologically plausible and supported by observational research. And improving hydration habits is one of the lowest-cost, lowest-risk interventions available for a condition that is expensive, difficult to treat, and significantly disabling.
The most directly relevant study was published in the European Journal of Neurology by Spigt and colleagues. Participants with recurrent headache were randomized to increase their daily water intake by 1.5 liters above baseline or to follow a control condition without hydration changes. Those in the water group reported significantly fewer total hours of headache per month and subjectively rated their headache severity as lower, though the reduction in attack frequency did not reach statistical significance in this relatively small study.
A broader body of research on dehydration and neurological function supports the biological plausibility of the hydration-migraine relationship. Mild dehydration, defined as a body water loss of one to two percent, produces measurable changes in mood, cognitive performance, and pain sensitivity in healthy adults.
The brain is particularly sensitive to changes in fluid status. It is approximately seventy-three percent water, and it is housed in a rigid bony compartment that prevents expansion. When systemic dehydration reduces the volume of cerebrospinal fluid that cushions the brain, the brain may shift slightly within the skull, pulling on pain-sensitive dural structures and potentially activating the trigeminal pathways involved in migraine pain.
Beyond the structural hypothesis, dehydration increases plasma osmolality, which activates osmoreceptors in the hypothalamus. Given the role of the hypothalamus in migraine initiation, the response of this region to dehydration may contribute to attack risk through pathways that are independent of the structural mechanism.
Hydration needs vary significantly between individuals based on body size, activity level, climate, dietary composition, and individual physiology. More useful individual guidance includes monitoring urine color as a practical hydration indicator, with pale yellow urine suggesting adequate hydration and darker urine suggesting the need for increased intake.
Morning hydration deserves specific attention. During sleep, the body goes several hours without fluid intake. People who wake without drinking water and immediately consume caffeine-containing beverages may begin the day in a mildly dehydrated state. Some people with migraine report that drinking a full glass of water immediately upon waking reduces the frequency of morning attacks.
Plain water is the most straightforward hydration intervention, but the role of electrolytes in hydration quality is relevant for people who are physically active or who live in hot climates. Magnesium in particular has an established relationship with migraine, and several clinical trials have found that magnesium supplementation reduces migraine frequency.
People with migraine who exercise regularly, sweat heavily, or have diets low in magnesium-rich foods may benefit from attention to both hydration and electrolyte balance alongside each other.
Hydration should be understood as one variable in the migraine trigger system rather than a primary intervention. For someone whose migraine is driven primarily by hormonal factors, sleep disruption, or medication overuse, improving hydration is unlikely to produce a meaningful reduction in attack frequency. For someone who is habitually underhydrated and whose tracking data reveals a pattern of attacks following periods of low fluid intake, improving hydration may produce a noticeable reduction.
The only reliable way to know which of these descriptions applies to a given individual is to track hydration alongside attack data and look for patterns. That identification, rather than the generic recommendation to drink more water, is what makes hydration advice actually useful.
