Riboflavin, the common name for vitamin B2, is a water-soluble B vitamin that plays essential roles in cellular energy production. And nutrients that support mitochondrial function, including riboflavin and coenzyme Q10, have shown some benefit as migraine preventives in clinical trials.
Riboflavin is a cofactor for two enzymes involved in the mitochondrial electron transport chain: flavin mononucleotide and flavin adenine dinucleotide. These flavin coenzymes are essential for oxidative phosphorylation, the process by which mitochondria produce the ATP that powers cellular function. If mitochondrial energy production in brain cells is suboptimal, supplementing riboflavin at doses above normal dietary intake may support the electron transport chain and improve the energetic buffer available to cortical neurons.
The most influential clinical trial of riboflavin for migraine prevention was conducted by Schoenen and colleagues and published in Neurology in 1998. In this randomized, placebo-controlled trial, 400 milligrams of riboflavin per day was associated with a significant reduction in migraine attack frequency and headache days compared to placebo over a sixteen-week treatment period. Fifty-nine percent of riboflavin-treated patients achieved a greater than fifty percent reduction in attack frequency, compared to fifteen percent in the placebo group.
Subsequent trials have produced more mixed results. A pediatric trial found riboflavin no more effective than placebo in children and adolescents with migraine, though the dose and duration may have been insufficient. Overall, the evidence suggests that 400 milligrams per day may be the effective dose range for adults, with effects emerging over several weeks to months of consistent use.
Riboflavin is remarkably safe. It is a water-soluble vitamin and excess intake is excreted in urine, which turns bright yellow at supplemental doses — a harmless and expected side effect that patients should be informed about before starting supplementation. No serious adverse effects have been attributed to riboflavin supplementation at the doses used in migraine trials.
This favorable safety profile, combined with the low cost of riboflavin supplements, makes it an appropriate consideration for people with migraine who are looking for adjunctive preventive options that do not add medication burden or side effect risk. It is included in the American Academy of Neurology and American Headache Society evidence-based guidelines for migraine prevention with a Grade B recommendation.
The dose with the strongest evidence is 400 milligrams per day, taken once daily or in divided doses with meals. Riboflavin is best absorbed with food. The yellow discoloration of urine is expected and not a sign of excess or toxicity.
A trial period of at least three months is appropriate before assessing effectiveness, as the mitochondrial effects of riboflavin supplementation develop over time. Riboflavin can be used alongside other preventive medications and supplements, including magnesium, without known interactions.
As with other preventive approaches, response to riboflavin is individual. Some people experience significant reductions in attack frequency with riboflavin supplementation; others do not respond. Tracking attack frequency before and during supplementation provides the data needed to assess whether riboflavin is contributing to improvement. A headache diary maintained consistently over the three-month trial period provides the most reliable basis for evaluating response.
