Retinal Migraine: A Rare Cause of One-Eyed Vision Loss

Retinal migraine is a rare and specific migraine subtype involving temporary visual disturbance or vision loss confined to a single eye, caused by changes affecting the eye itself rather than the brain's visual processing centers. Because sudden vision changes in one eye can also signal serious, urgent conditions, retinal migraine is a diagnosis that requires careful evaluation rather than assumption, even for someone with an established migraine history.

What Distinguishes Retinal Migraine

The defining feature of retinal migraine is that visual symptoms are genuinely confined to one eye, confirmed by covering each eye individually during an episode. This differs fundamentally from the far more common visual aura seen in typical migraine with aura, which originates in the brain's visual cortex and therefore affects the visual field in both eyes, even when initially perceived as one-sided.

According to diagnostic criteria, retinal migraine involves fully reversible monocular visual symptoms, which can include scintillations similar to those seen in typical aura, scotomas, or complete vision loss, occurring in association with migraine, and confirmed where possible through visual field testing during an episode, along with normal ophthalmological examination between episodes.

How Rare It Actually Is

Retinal migraine is considerably rarer than typical migraine with visual aura. Precise prevalence figures are difficult to establish because the condition is uncommon enough that large-scale studies are limited, and because the diagnosis requires careful confirmation to distinguish it from other causes of monocular visual disturbance, meaning some cases informally labeled as retinal migraine by patients may not meet the formal diagnostic criteria upon evaluation.

Because of its rarity and its overlap in name and description with the more common and more loosely used term "ocular migraine," retinal migraine is frequently over-diagnosed informally and under-diagnosed formally, meaning many people describe their bilateral visual aura as retinal migraine without it being confirmed as monocular, while genuine cases may not always be recognized as a distinct condition without specialist evaluation.

The Underlying Mechanism

The proposed mechanism behind retinal migraine involves vasospasm, a temporary narrowing of blood vessels supplying the retina or optic nerve, reducing blood flow and producing the visual symptoms. This is a meaningfully different mechanism from the cortical spreading depression believed to underlie typical visual aura, which is part of why the two conditions, despite being colloquially grouped together, are treated as clinically distinct.

Why Evaluation Is Essential

Sudden vision loss or disturbance confined to one eye has a broad differential diagnosis, many of which are considerably more common than retinal migraine and some of which represent genuine emergencies requiring immediate treatment to preserve vision or rule out stroke. These include retinal artery or vein occlusion, retinal detachment, optic neuritis, and in the context of a possible stroke, amaurosis fugax, a form of transient monocular vision loss caused by temporary blockage of blood flow to the retina from a vascular source rather than migraine.

Because of this overlap, retinal migraine is a diagnosis of exclusion, meaning it should only be confirmed after an eye care professional has ruled out structural, vascular, and other ophthalmological causes, ideally including examination during or shortly after an episode when possible, since the eye typically appears entirely normal between episodes, which can make diagnosis without a documented episode more difficult.

Recognized Risk Considerations

Some research has raised questions about a potential association between retinal migraine and an elevated risk of retinal vascular events over time, given the vasospastic mechanism involved, though this remains an area of ongoing study rather than settled, high-certainty evidence. This is part of why ongoing monitoring by an ophthalmologist, in addition to management by a neurologist or headache specialist, is often recommended for confirmed cases rather than managing the condition through headache-focused care alone.

Symptoms and Pattern

A retinal migraine episode typically involves visual disturbance, dimming, or complete vision loss in one eye, lasting anywhere from a few minutes to about an hour, followed by full visual recovery. A headache, when present, usually follows the visual symptoms and shares features with typical migraine, though as with other aura-related presentations, headache is not universal in every episode.

What to Do During and After an Episode

Anyone experiencing new, one-sided visual symptoms for the first time should seek prompt medical evaluation rather than assuming a migraine cause, given how many alternative explanations require urgent treatment. For someone with an already confirmed retinal migraine diagnosis, having a clear plan established with their physician for what to do during a typical episode, and what would represent a change from that pattern requiring re-evaluation, provides useful clarity for a condition that can otherwise feel frightening each time it recurs.

Keeping a record of episode duration, extent of vision loss, whether a headache followed, and any potential triggers supports both ongoing management and the differential diagnosis process, particularly if episodes are still being evaluated for a first-time confirmed diagnosis.
