Migraine and Tinnitus: Auditory Sensitivity and Comorbidity

Tinnitus, the perception of sound in the absence of an external acoustic stimulus, and migraine co-occur more frequently than expected by chance, and both conditions involve heightened sensitivity of sensory processing systems that share neurological substrates. For people who experience both tinnitus and migraine, the conditions can interact in ways that compound the burden of each — migraine attacks may transiently worsen tinnitus, and the constant presence of tinnitus may maintain a level of auditory and neurological arousal that contributes to migraine vulnerability.

The Prevalence of Tinnitus in Migraine

Studies have found elevated rates of tinnitus in people with migraine compared to headache-free populations, with some research documenting rates two to three times higher than expected. The association is particularly strong for migraine with aura and for vestibular migraine, a migraine subtype characterized by vertigo and dizziness that has particularly strong connections to inner ear and auditory processing systems.

The Auditory System and Migraine

Sound sensitivity, known as phonophobia, is one of the defining features of migraine and is present in the majority of people with the condition during attacks. The pathway through which sound worsens migraine involves the auditory cortex and its connections to limbic and brainstem pain-modulating systems, as well as the trigeminal nerve branches that innervate the middle ear and tympanic membrane.

Between attacks, the migraine brain demonstrates elevated auditory cortex excitability that persists in the interictal period. This cortical hyperexcitability may increase vulnerability to tinnitus by amplifying internal auditory signals that a normally excitable cortex would suppress.

The Trigeminal-Auditory Connection

The trigeminal nerve innervates the middle ear through the tensor tympani muscle, which regulates middle ear stiffness and acoustic transmission. Trigeminal activation during migraine may alter tensor tympani function and change middle ear acoustic properties in ways that influence both sound sensitivity and tinnitus perception. This trigeminal-auditory connection provides a mechanistic pathway through which migraine pathophysiology could directly affect tinnitus.

Vestibular Migraine and Auditory Symptoms

Vestibular migraine, a recognized migraine subtype, is characterized by episodic vertigo and dizziness that may occur with or without headache. People with vestibular migraine frequently report auditory symptoms including tinnitus, ear fullness, and fluctuating hearing that may overlap with or mimic Meniere's disease. The distinction between vestibular migraine, Meniere's disease, and migraine with vestibular and auditory features requires careful clinical evaluation.

Management Considerations

Management of co-occurring tinnitus and migraine involves addressing both conditions without allowing treatments for one to worsen the other. Discussing both conditions with providers who are aware of both diagnoses ensures coordinated care. Cognitive behavioral therapy, which has evidence for both tinnitus distress and migraine disability, represents a potentially valuable shared intervention. Sound therapy used for tinnitus management should be evaluated for its effects on migraine light and sound sensitivity before implementation.
