Post-traumatic stress disorder and migraine co-occur at rates substantially higher than would be expected by chance. Research in veteran populations, civilian trauma survivors, and general clinical samples consistently documents elevated migraine prevalence in people with PTSD and elevated PTSD rates in people with chronic migraine. The relationship reflects genuine shared neurobiological vulnerability rather than the coincidence of two common conditions or the stress of coping with one causing the other.
Understanding the mechanisms connecting PTSD and migraine, how each condition influences the other's course, and what treatment implications arise from their co-occurrence helps people with both conditions and their healthcare providers take a more integrated approach to management.
Studies of veteran populations have found migraine prevalence rates of thirty to fifty percent in veterans with PTSD, substantially higher than the general population prevalence. In civilian populations with PTSD following sexual assault, accidents, and other traumatic events, migraine rates are similarly elevated. Conversely, people with chronic migraine have higher rates of PTSD than people with episodic migraine or no headache disorder, suggesting that the burden and unpredictability of chronic pain itself may be traumatizing for some individuals.
The strongest associations appear in people who experienced traumatic events involving head or neck injury, interpersonal violence, or sexual assault — categories of trauma that are particularly associated with both PTSD and chronic pain conditions.
PTSD and migraine share several neurobiological systems. The hypothalamic-pituitary-adrenal axis dysregulation that characterizes PTSD, with its patterns of altered cortisol reactivity and abnormal stress hormone responses, creates a neurological environment that may lower the migraine threshold through the same mechanisms by which chronic stress affects migraine vulnerability.
Amygdala hyperreactivity, a cardinal feature of PTSD that underlies the heightened threat detection and emotional reactivity of the condition, connects to migraine through shared pathways in limbic-brainstem circuits that modulate both emotional processing and pain sensitivity. Serotonergic dysfunction is implicated in both conditions — reduced serotonergic signaling contributes to the fear extinction deficits of PTSD and to the altered pain modulation of migraine through overlapping pathways.
Allodynia, the perception of normally non-painful stimuli as painful, is a feature of migraine with central sensitization and also occurs in PTSD as part of the heightened sensory reactivity that characterizes the condition. In PTSD, allodynia reflects the sensitization of sensory processing systems by repeated trauma-related activation of stress circuits. In migraine, it reflects central sensitization of trigeminal and thalamic pain pathways during and after attacks.
The overlap in allodynia between the two conditions suggests common sensitization mechanisms and may contribute to the elevated migraine burden in people with PTSD through an already-sensitized pain system that requires less additional input to reach the threshold for a full migraine attack.
PTSD influences migraine through several mechanisms. The hypervigilance of PTSD maintains the nervous system in a state of chronic arousal that mimics the physiological effects of sustained stress, with elevated cortisol, altered autonomic tone, and disrupted sleep. Each of these features independently increases migraine vulnerability.
Sleep disturbance is particularly significant. PTSD produces severe and persistent sleep disruption through nightmares, hyperarousal at night, and conditioned fear responses to sleep itself. The resulting sleep fragmentation and deprivation maintain the migraine nervous system in a state of heightened vulnerability through the mechanisms of sleep deprivation as a migraine trigger.
Trauma-focused therapies including prolonged exposure, cognitive processing therapy, and EMDR that reduce PTSD severity may also reduce migraine frequency through their effects on the shared neurobiological mechanisms. Medications with evidence for both PTSD and migraine prevention, including amitriptyline and prazosin, may offer advantages in people with both conditions. A coordinated treatment approach involving both a mental health provider experienced in trauma and a headache specialist produces better outcomes than treating each condition independently.
