Biofeedback is a behavioral technique in which real-time information about a person's own physiological processes is displayed to them, allowing them to learn to voluntarily control those processes through focused attention and relaxation. In migraine, biofeedback has one of the strongest evidence bases of any non-pharmacological treatment, with multiple randomized controlled trials demonstrating reductions in attack frequency comparable to some preventive medications.
Despite this evidence base, biofeedback remains underutilized in migraine management, partly because it requires specialized training and equipment, partly because it is less well known than pharmacological options, and partly because its mechanism is less intuitive than taking a pill. Understanding how biofeedback works, what the evidence shows, and who is most likely to benefit helps people with migraine and their healthcare providers evaluate whether it belongs in the management plan.
Biofeedback works by making normally invisible physiological processes visible in real time. A sensor attached to the body measures a physiological variable — skin temperature, muscle tension, heart rate variability, or electroencephalographic activity — and feeds this information back to the person through a display, a sound, or a graph that updates continuously.
The person then attempts to change the physiological variable in the desired direction using mental and physical strategies including focused relaxation, visualization, breathing techniques, and attention control. The real-time feedback allows them to know immediately whether their efforts are producing the desired change, which accelerates the learning process. Over multiple training sessions, people develop the ability to voluntarily control the physiological variable without the feedback display.
Temperature biofeedback, also called thermal biofeedback or hand warming, is the most extensively studied form for migraine. It uses a temperature sensor on the fingertip to display skin temperature, and trains people to voluntarily increase finger temperature through peripheral vasodilation. The physiological rationale is that voluntary peripheral vasodilation may be associated with a complementary reduction in central vascular tone that could reduce meningeal vasodilation and lower the migraine threshold.
Electromyographic biofeedback measures muscle electrical activity in the frontalis, temporalis, or trapezius muscles and provides feedback that allows voluntary reduction of pericranial and cervical muscle tension. This form is particularly relevant for people whose migraine is associated with prominent muscle tension and neck pain.
A comprehensive meta-analysis and the American Academy of Neurology evidence-based guidelines both conclude that biofeedback has Grade A evidence for the prevention of migraine, meaning it is established as effective based on multiple well-designed randomized controlled trials. Effect sizes in the range of a forty to fifty percent reduction in attack frequency have been reported, comparable to some pharmacological preventive treatments.
Biofeedback appears particularly effective when combined with relaxation training and when used in people with high physiological reactivity to stress, though benefits have been documented across a range of migraine patient characteristics.
Biofeedback for migraine is delivered by licensed psychologists, biofeedback therapists, or other trained clinicians. Treatment typically involves six to ten sessions of thirty to sixty minutes each, with home practice between sessions using portable biofeedback devices. The Biofeedback Certification International Alliance maintains a directory of certified biofeedback practitioners.
Insurance coverage for biofeedback varies. Some plans cover it for headache disorders, and prior authorization may be required. Home biofeedback devices are available for purchase and may be appropriate for ongoing practice after an initial course of in-person training.
Biofeedback tends to produce the greatest benefit in people who are motivated to practice consistently, who have identifiable physiological reactivity to stress, and who prefer non-pharmacological approaches or need to supplement inadequate pharmacological control. It requires more active patient engagement than taking a medication, but the skills learned through biofeedback training are durable and can be applied across the lifespan without ongoing cost or side effects.
