Effective migraine treatment is not one thing. It is a combination of strategies, individualized to the person, adapted over time as the condition changes, and informed by data about what is actually working. The people who manage migraine most successfully are rarely those who found one medication and used it indefinitely without adjustment. They are the ones who understood the treatment landscape, worked with clinicians who knew it too, and built a plan that addressed both acute attacks and the underlying frequency of the condition.

Migraine treatment operates on two parallel tracks. Acute treatment addresses attacks as they occur. Preventive treatment reduces how often attacks happen and how severe they are when they do.
Both are necessary components of comprehensive care for most people with frequent or disabling migraine. Relying exclusively on acute treatment when preventive treatment is indicated leads to escalating medication use, reduced medication effectiveness over time, and the risk of medication overuse headache. Relying exclusively on preventive treatment without an adequate acute strategy leaves attacks without effective management when they do occur.
Acute treatment is medication or other intervention taken at the onset of a migraine attack with the goal of reducing its severity and duration. The effectiveness of acute treatment depends significantly on timing. Research consistently shows that treating migraine early in the attack, before central sensitization develops and before the headache becomes severe, produces better pain relief, faster resolution, and lower rates of headache recurrence than waiting until the pain is at its worst.
Non-specific analgesics including ibuprofen, aspirin, and acetaminophen are the most accessible acute treatments and are effective for mild to moderate migraine attacks in many people. Triptans are migraine-specific acute medications that work by activating serotonin receptors in trigeminal nerve terminals, reducing CGRP release, and promoting vasoconstriction of dilated meningeal blood vessels. They are contraindicated in people with cardiovascular disease, uncontrolled hypertension, and some other conditions.
Gepants, the small molecule CGRP receptor antagonists, represent the newest class of acute migraine medications. Rimegepant and ubrogepant have demonstrated efficacy in clinical trials and offer an alternative for people who cannot use triptans. Antiemetics including metoclopramide and ondansetron address nausea and vomiting during attacks and also improve gastric motility, which enhances absorption of co-administered oral medications.

Preventive treatment is taken regularly, whether or not an attack is occurring, with the goal of reducing attack frequency, severity, and duration over time. It is recommended when migraine occurs on four or more days per month, when attacks are severely disabling even when infrequent, when acute medications are being overused, or when the person prefers to reduce overall attack burden.
The most important practical consideration in preventive treatment is time. Preventive medications require eight to twelve weeks of consistent use at an adequate dose before their full effect is apparent. People who discontinue a preventive medication after three or four weeks because they see no immediate improvement may be stopping before the medication has had time to work.
Established oral preventive medications include beta blockers, tricyclic antidepressants, anticonvulsants, and calcium channel blockers. CGRP-targeted preventive medications represent the most significant advance in migraine prevention in decades. Onabotulinumtoxin A is approved specifically for the prevention of chronic migraine in adults.

Behavioral treatments have evidence for migraine prevention that is often underappreciated relative to pharmacological options. Cognitive behavioral therapy, biofeedback, relaxation training, and mindfulness-based interventions all have clinical trial evidence supporting their use. The American Headache Society recognizes behavioral treatments as evidence-based components of comprehensive migraine management.
Regular moderate aerobic exercise has evidence for reducing migraine frequency comparable in some studies to first-line preventive medications. Consistent sleep timing, adequate hydration, regular meal timing, and stress management all have supporting evidence and present no meaningful risk.

Medication overuse headache is one of the most important and preventable complications of migraine treatment. It develops when acute medications are used on ten or more days per month for triptans and combination analgesics, or fifteen or more days per month for simple analgesics, over a period of three months.
Prevention is straightforward in principle and challenging in practice. It requires keeping acute medication use below the overuse threshold, which in turn requires either reducing attack frequency through preventive treatment or accepting that some attacks will be managed with less medication than would be used without the constraint.

An effective migraine treatment plan has several components. A reliable acute treatment taken early in the attack. An antiemetic strategy for attacks with significant nausea. A preventive treatment if attack frequency meets the threshold for prevention. Behavioral interventions targeted at the specific factors most relevant to the individual's pattern. A tracking system that generates the data needed to assess whether the plan is working and what needs to be adjusted.
A treatment plan that is not evaluated against data is a guess. A plan that is regularly reviewed against prospective tracking data is a strategy. The difference between the two determines, more than any individual treatment choice, whether migraine is managed or simply endured.
A neurologist or headache specialist should be involved in migraine management when attacks are frequent or disabling, when two or more preventive treatments have failed, when chronic migraine is present, when the diagnosis is uncertain, or when the clinical picture is complex.
