Migraine and Anxiety: The Neurological Overlap Explained

Anxiety and migraine co-occur at rates that far exceed what chance would predict. Research consistently finds that people with migraine are two to five times more likely to have an anxiety disorder than people without migraine, and people with anxiety disorders are at elevated risk for migraine compared to the general population. This bidirectional relationship is not coincidental and is not simply explained by the psychological burden of living with a painful chronic condition. It reflects shared neurobiological mechanisms that make the two conditions more likely to develop together and more likely to influence each other's severity and course.

Understanding the relationship between migraine and anxiety has practical implications for both conditions. Anxiety can increase migraine frequency. Migraine can worsen anxiety. Treatments that address one may benefit both. And the interaction between the two conditions requires a more integrated approach to management than treating each in isolation.

The Epidemiological Evidence

Large population-based studies have consistently documented the co-occurrence of migraine and anxiety disorders. Generalized anxiety disorder, panic disorder, and phobic disorders are all significantly more prevalent in people with migraine than in headache-free populations. The relationship is particularly strong for migraine with aura, which shows higher rates of anxiety comorbidity than migraine without aura.

The bidirectionality of the relationship has been demonstrated in prospective studies showing that anxiety at baseline predicts the new onset of migraine, and that migraine at baseline predicts the new onset of anxiety disorders. This temporal bidirectionality suggests shared vulnerability factors rather than simple causation in one direction.

Shared Neurobiological Mechanisms

Several neurobiological systems are implicated in both migraine and anxiety, providing a mechanistic basis for their co-occurrence.

The serotonergic system is the most extensively studied. Serotonin regulates mood, anxiety, pain sensitivity, and the modulation of the trigeminovascular system. Dysfunction in serotonergic signaling is implicated in anxiety disorders through effects on amygdala reactivity and limbic circuit function, and in migraine through effects on brainstem pain modulation and trigeminal sensitization. The same serotonergic dysfunction may increase vulnerability to both conditions simultaneously.

The hypothalamic-pituitary-adrenal axis, the body's primary stress response system, is dysregulated in both anxiety and migraine. Chronic stress activation of this axis increases cortisol output, alters neuronal excitability, and promotes inflammatory processes in the nervous system. These effects can lower the migraine threshold while also maintaining the state of heightened arousal characteristic of anxiety.

Glutamatergic excitability is elevated in both conditions. The migraine brain is characterized by cortical hyperexcitability, and glutamatergic hyperactivity in the amygdala and prefrontal cortex is implicated in anxiety disorders. Genetic variants affecting glutamate receptors and signaling have been associated with both migraine and anxiety, pointing to shared genetic vulnerability.

How Anxiety Influences Migraine

Anxiety influences migraine through several mechanisms. Chronic anxiety maintains the hypothalamic-pituitary-adrenal axis in a state of activation that increases cortisol and promotes the inflammatory and excitatory processes that lower the migraine threshold. Anxiety-related sleep disturbance, a common feature of generalized anxiety disorder and panic disorder, disrupts circadian regulation and is itself one of the most consistent migraine triggers.

Anxiety also influences how people respond to migraine. People with anxiety are more likely to anticipate attacks with fear, more likely to catastrophize about their severity, and more likely to develop avoidance behaviors around activities or situations associated with previous attacks. These psychological responses can maintain heightened neurological arousal that further increases attack frequency in a self-perpetuating cycle.

How Migraine Influences Anxiety

Migraine influences anxiety through the unpredictability of attacks and the impact of that unpredictability on daily planning and functioning. The inability to predict when a disabling attack will occur generates anticipatory anxiety that is rational in the circumstances but neurologically costly. Anticipatory anxiety about migraine has been shown to predict increased attack frequency, creating a cycle in which fear of the next attack contributes to its occurrence.

The interictal period in migraine, the time between attacks, is not neurologically normal. Research has documented hyperexcitability of sensory processing systems during the interictal period that may contribute to the heightened arousal and reactivity characteristic of anxiety.

Treatment Implications

The co-occurrence of migraine and anxiety has treatment implications that argue for integrated rather than siloed management. Several medications have evidence for both conditions. Amitriptyline, a tricyclic antidepressant with both analgesic and anxiolytic effects, is used as a migraine preventive and as a treatment for anxiety. Propranolol, a beta-blocker used in migraine prevention, reduces physiological symptoms of anxiety including heart rate and tremor.

Cognitive-behavioral therapy, which is evidence-based for anxiety disorders, has also shown efficacy for migraine management through its effects on pain catastrophizing, stress reactivity, and behavioral avoidance. Addressing both conditions simultaneously through integrated behavioral and pharmacological treatment is more effective than treating each condition independently.

Massage therapy and other body-based approaches also target this same pathway, through a different mechanism than CBT or medication. This may be worth particular attention for people who'd prefer to avoid pharmaceutical approaches where possible, or who've already tried medications for anxiety or migraine without meaningful relief. By reducing muscle tension in the neck, shoulders, and upper back and helping regulate autonomic nervous system activity, regular massage may lower the physiological arousal that links anxiety to migraine — the same hypothalamic-pituitary-adrenal (HPA) axis activity discussed above. Evidence specifically for massage in this context is still limited, but small controlled trials have found reduced migraine frequency with regular massage therapy, and the mechanism is consistent with what's already understood about anxiety-driven attacks. For someone whose anxiety shows up as physical tension as much as racing thoughts — or who's run out of pharmaceutical options that actually helped — this is a nonpharmacological avenue worth discussing with a provider.
