Hemiplegic migraine is one of the rarer and more alarming forms of migraine with aura, distinguished by temporary motor weakness on one side of the body. Because that weakness closely resembles the presentation of a stroke, hemiplegic migraine is a condition that demands careful evaluation, clear diagnostic criteria, and, for those who live with it, a well-understood management plan.
Most forms of migraine aura involve visual disturbances, sensory changes such as tingling, or speech difficulty. Hemiplegic migraine adds a distinct and more serious symptom to that list: temporary motor weakness, or hemiparesis, affecting one side of the body as part of the aura phase. This weakness can range from mild clumsiness to a pronounced inability to move the affected arm or leg, and it is typically accompanied by other aura symptoms, most often visual or sensory, occurring alongside or before the motor symptoms.
According to research on the condition, motor aura symptoms in hemiplegic migraine usually last under seventy-two hours, though duration varies considerably between individuals and even between attacks in the same person. Some episodes resolve within an hour, while others persist for a day or more.
Hemiplegic migraine occurs in two forms. Familial hemiplegic migraine, or FHM, is diagnosed when at least one first or second degree relative has experienced attacks meeting the same diagnostic criteria, reflecting the condition's autosomal dominant genetic inheritance pattern. Specific gene mutations, most commonly affecting the CACNA1A, ATP1A2, and SCN1A genes, have been identified in many familial cases, all of which are involved in ion transport within neurons.
Sporadic hemiplegic migraine, or SHM, describes cases with no known family history, though a genetic basis may still be present without a documented family pattern. Research indicates that hemiplegic migraine overall is genuinely rare, with prevalence estimates around 0.01 percent of the population, and onset most commonly occurring in adolescence, typically between ages twelve and seventeen. Women are affected more frequently than men, with studies reporting female to male ratios ranging from roughly two and a half to one, up to over four to one.
Sudden one-sided weakness is a hallmark warning sign of stroke, which means a first hemiplegic migraine episode, particularly in someone with no prior diagnosis, frequently results in an emergency room visit and stroke workup. This is appropriate and necessary. Hemiplegic migraine is fundamentally a diagnosis of exclusion, meaning stroke, transient ischemic attack, seizure with postictal weakness, and other neurological emergencies must be ruled out before hemiplegic migraine can be confirmed, especially at first presentation.
Distinguishing features that support a hemiplegic migraine diagnosis over stroke include a gradual onset and spread of symptoms over minutes rather than the sudden onset typical of stroke, a personal or family history of migraine, the presence of accompanying visual or sensory aura symptoms, and eventual full resolution of the weakness. Even with these distinguishing features, any first episode of hemiplegic-migraine-like symptoms warrants emergency evaluation.
Beyond motor weakness, hemiplegic migraine attacks can involve confusion, impaired consciousness ranging from mild to significant, fever, and in more severe cases, cerebellar symptoms such as difficulty with coordination and balance. Some individuals experience these additional features only during their most severe attacks, while others experience them consistently.
The headache phase, when present, typically follows the aura and motor symptoms, though headache is not universal in every attack. Some individuals experience the aura and motor weakness without a subsequent significant headache, which can add to diagnostic complexity, since the presentation may not immediately register as migraine-related at all.
Diagnosis relies on a detailed clinical history, family history assessment, and the ICHD-3 diagnostic criteria for hemiplegic migraine, which require at least two attacks featuring fully reversible motor weakness alongside other aura symptoms. Neuroimaging, typically MRI, is usually performed, especially at initial presentation, both to rule out other causes and, in some cases, to identify supportive findings. Genetic testing may be considered in familial cases, particularly when the diagnosis affects family planning or when atypical features are present, though a negative genetic test does not rule out the diagnosis, since not all causative mutations have been identified.
Treatment for hemiplegic migraine differs in an important way from typical migraine treatment. Triptans and other vasoconstrictive medications, commonly used for standard migraine with aura, are generally avoided or used with significant caution in hemiplegic migraine due to theoretical concerns about vascular effects during a condition that already involves complex neurological and vascular changes. Acute treatment instead often focuses on supportive care, and in more severe attacks, medical evaluation for symptom management.
Preventive treatment is frequently considered given the severity and disruption of attacks, with options including certain anticonvulsants and calcium channel blockers, chosen partly based on the underlying genetic mechanism when known. Given the specialized nature of this condition, care under a headache specialist or neurologist experienced with hemiplegic migraine is strongly recommended rather than management through general primary care alone.
